Editorial policy

Our standard is to show the source, explain the applicable coverage route and distinguish a documented rule from an estimate or recommendation.

Sources

CMS and Medicare.gov establish Medicare program rules. Current prescribing information establishes drug-specific indications and instructions. Manufacturer pages establish their published offers, subject to full terms and eligibility. HealthCare.gov explains Marketplace prescription coverage and appeals. KFF research is cited with its methods and data year; it is not presented as an official CMS enrollment count.

Dates and review scope

An “Updated” date marks a substantive revision. A source-check date applies to the stated topic, not a blanket review of the site. We do not change dates merely to make an unchanged article look fresh. Older articles may need checking against the current linked source before you act.

Medical review

No clinician review is credited for the pages in this edition. A future medical-review credit must identify the actual reviewer, credentials, review date and reviewed scope. An editorial source check is different from clinician review. Drug selection, diagnosis, dosing and treatment changes belong with your own clinician.

AI assistance and editorial responsibility

AI tools assist with research, drafting and code. Their output is checked against identified sources; unsupported claims are removed or labeled as unknown. Automation can flag a source change or prepare a draft, but it does not authorize publishing a medical claim or certifying a page as reviewed.

Paid relationships
