BMI 30: which tier am I in?

Short answer: A BMI of 30 to 34.9 qualifies you for Medicare's $50/month GLP-1 Bridge with any one of six conditions: the Tier 2 set (chronic kidney disease stage 3a+, heart failure, or uncontrolled high blood pressure) or the Tier 3 set (prediabetes, prior heart attack or stroke, or symptomatic PAD), since Tier 3's threshold of 27 includes you.

Six ways to qualify at BMI 30

BMI 30 is about 186 lbs at 5'6", 197 lbs at 5'8", or 209 lbs at 5'10". At this range you have the widest set of qualifying paths in the whole program: CKD stage 3a or higher, heart failure (including HFpEF), high blood pressure not controlled by medication, prediabetes, a previous heart attack or stroke, or symptomatic peripheral artery disease.

Practical implication: at BMI 30+, one ordinary lab panel (A1c for prediabetes, eGFR for kidney function) checks two of the six paths at once. If you haven't had bloodwork in the past year, that single appointment may answer the whole eligibility question.

Common questions

Can I qualify at BMI 32 with no other conditions?

No — BMI alone qualifies only at 35+. Between 30 and 34.9 you need one of the six qualifying conditions, though several (like prediabetes and early CKD) are common and frequently undiagnosed.

Which qualifying condition is most common?

Prediabetes, by a wide margin — roughly half of older adults meet its definition, most without knowing. An A1c test settles it.
