Short answer: Chronic kidney disease at stage 3a or higher is a Tier 2 qualifying condition: combined with a BMI of 30 or more, it makes you eligible for Medicare's $50/month GLP-1 Bridge program. Stage 3a begins when kidney function (eGFR) falls to about 59 or below — a level identified from routine bloodwork.

Checking whether your CKD qualifies
CKD staging runs from 1 (mild) to 5 (kidney failure), based mostly on eGFR, a number calculated from the creatinine in a standard blood panel. Stage 3a means eGFR 45–59; anything at stage 3a or beyond qualifies for Tier 2. If you've been told you have 'mild kidney disease' or 'reduced kidney function,' ask your doctor specifically: 'What is my eGFR, and what stage am I?'
Many people learn they have stage 3 CKD only when they ask — it's frequently noted in lab results without being emphasized, especially when stable. For this program, that overlooked lab line could be worth hundreds of dollars a month.
Why GLP-1s and kidney disease intersect
CMS lists CKD stage 3a or above as a qualifying condition at starting BMI 30 or higher. Your clinician confirms the diagnosis and whether treatment is appropriate; this guide cannot establish why CMS selected that criterion.

Does stage 2 CKD qualify?
No — the criterion is stage 3a or higher (eGFR roughly 59 or below). Stage 2 alone doesn't qualify under Tier 2, but check the other qualifying conditions; prediabetes at BMI 27+ is a common alternative path.
I'm on dialysis. Am I eligible?
Advanced CKD meets the staging criterion, but whether a GLP-1 is clinically appropriate on dialysis is a decision for your kidney team. Eligibility runs through their prior authorization.
What BMI do I need with CKD?
30 or higher. If your BMI is 27–29.9, CKD alone doesn't qualify you — but prediabetes, a prior heart attack or stroke, or symptomatic PAD would.
