Short answer: Once your prior authorization is approved, the Bridge works like a normal monthly prescription: each 28-30 day fill is $50 at a participating pharmacy. The things worth understanding are the edges — dose-change fills, moving or traveling, and the program's fixed end date.

Routine refills, and the edges
Supply depends on formulation: CMS includes weekly injections and daily tablets in 28-day or 30-day fills. Confirm refill timing with the pharmacy. Dose changes during titration are new strengths, not new authorizations — the $50 applies to each fill either way. Changing pharmacies is fine (the program is nationwide through its central processor); the practical constraint is stock, especially Zepbound KwikPens — call ahead when traveling or relocating.
Ask the prescriber and pharmacist what to do about missed doses or interrupted treatment for your exact formulation. Do not assume every product has the same restart instructions.
Continuation and the 2027 horizon
The demonstration currently ends December 31, 2027. Nothing about your refills changes before then, but the end date should shape the conversation you have with your prescriber about goals and exit strategy: what maintenance looks like, whether a routing condition (like progressing to a diabetes diagnosis) would move you to Part D coverage, and what the self-pay fallback costs if the program sunsets without replacement. Can I get a 90-day supply?
The program is built around monthly fills at $50 each. Ask your pharmacy what quantities process under the Bridge — and be wary of any arrangement quoting other amounts.
What if my pharmacy is out of stock?
Any participating pharmacy can fill it — have your prescription transferred, and for Zepbound confirm KwikPen stock specifically before the transfer.
Does my authorization expire?
Authorizations carry their own terms; your prescriber's office sees renewal requirements. Flag it if a pharmacy says an authorization has lapsed — renewals are routine paperwork, not re-qualification from scratch.
