Monthly cadence: these are weekly injections dispensed in monthly quantities; your pharmacy manages timing like any maintenance drug. Dose changes during titration are new strengths, not new authorizations — the $50 applies to each fill either way. Changing pharmacies is fine (the program is nationwide through its central processor); the practical constraint is stock, especially Zepbound KwikPens — call ahead when traveling or relocating.
Gaps matter more with GLP-1s than most drugs: an interrupted course generally means restarting titration from a low dose, losing a month or more of therapeutic effect. Set refill reminders and don't run the tank empty before reordering.
Continuation and the 2027 horizon
The demonstration currently ends December 31, 2027. Nothing about your refills changes before then, but the end date should shape the conversation you have with your prescriber about goals and exit strategy: what maintenance looks like, whether a routing condition (like progressing to a diabetes diagnosis) would move you to Part D coverage, and what the self-pay fallback costs if the program sunsets without replacement. What if my pharmacy is out of stock?
