Where the extension question stands, July 2026

Three weeks into the program, the most common reader question is already about its end: will the Bridge run past December 31, 2027? The current answer is unambiguous — no extension or successor has been announced, and KFF's assessment is that there's presently 'no clear path forward' for Medicare GLP-1 weight-loss coverage after 2027.

The history explains the uncertainty. The Bridge exists because its designed successor — the BALANCE Model, which invited Part D plans to cover these drugs voluntarily — stalled when plan participation fell far short of CMS's threshold. The extension of the Bridge's own window from six to eighteen months was a symptom of that failure, not a signal of commitment. Any durable coverage now requires either different economics (cheaper drugs, plan reinsurance, a mandate) or congressional action on Part D's underlying weight-loss exclusion.

Our planning advice is deliberately boring and hasn't changed: treat the end date as real. Start therapy on its merits with maximum runway, have the exit conversation with your prescriber before it's urgent, and let policy upside be a surprise.
