Short answer: Zepbound starts at 2.5 mg weekly for four weeks, then steps to 5 mg, with further increases (7.5, 10, 12.5, up to 15 mg) as tolerated at four-week intervals. Each month's KwikPens are one $50 Bridge fill, and many patients maintain effectively at middle doses rather than the maximum.

How prescribers run the climb

The 2.5 mg month is an adaptation dose — meaningful effects typically begin from 5 mg up. From there, escalation is driven by response and tolerability: it's routine to hold at 5 or 7.5 mg if weight loss is progressing and side effects are manageable, and equally routine to climb further if progress stalls. The maximum 15 mg dose is not the default destination; it's the ceiling.

Program mechanics mirror Wegovy's: every fill is $50 whatever the strength, dose changes mean a new strength of KwikPen (keep the KwikPen requirement in mind at every change), and interrupted therapy generally restarts the titration.

How long until I'm at a 'working' dose?

Appetite effects commonly begin in the 5–7.5 mg range — month two or three. Full titration to the highest doses, if needed, takes five to six months.

Does a dose change require a new prior authorization?

Dose titration within an approved course is normal prescribing; your pharmacy processes the new strength as usual. Your prescriber's office handles any program paperwork if requirements change.

Which dose will I stay on?
